Elderly Person’s Medical Program[後期高齢者医療制度（75歳以上の人）]
People over 75 years of age (over 65 for those with certain disabilities) switch from National Health Insurance and sign up to the Elderly People Medical Program.

75歳（一定の障害がある方は65歳）以上の方は、国民健康保険などから離れて、後期高齢者医療制度の被保険者として医療を受けることになります。
Those Eligible（加入対象者）
People who meet the following requirements (excluding those who claim welfare benefits) will be eligible for this program.
- Everyone over 75 (from the date of the 75th birthday)
- Those between the ages of 65 and 74 and who have been recognized as having a certain disability by the Confederation of Public Services

次の要件に該当する方（生活保護受給者等を除く）が対象となります。
- 75歳以上の方すべて（75歳の誕生日当日から）
- 65歳から74歳の方で、申請により一定の障害があると広域連合によって認められた方
Application Procedure（申請方法）
In the case that a person between the ages of 65 and 74 seeks authorization to be insured due
to a certain disability, that person is required to process an application for disability recognition under the Elderly Person’s Medical Program (longevity medical program) at the City Hall reception.
