National Health Insurance (Those below 75 years of age)[国民健康保険（75歳未満の人）]

National Health Insurance is run on the basis of insurance premium payments and subsidizes hospital treatments in the case of diseases and injuries.
Any person who currently resides in Japan must be enrolled in one of medical insurance systems.  Those who are not enrolled in either a company-based health insurance program or the Eldery Person’s Medical Program, or those who do not claim income-support benefits must join the National Health Insurance Program provided by the city, town or village where they live.

　国民健康保険とは、あらかじめ保険料を支払うことで、病気やけがをしたときに安心して病院で治療を受けることができる制度です。
　日本国内に住所がある方は、必ず何らかの医療保険制度に加入しなければならないことになっています。会社の健康保険や後期高齢者医療制度に加入している方、生活保護を受けている方以外は、必ず住んでいる市町村の国民健康保険に入らなくてはいけません。

Anyone under the age of 75 and to whom the following apply must join National Health Insurance

• Those who are self-employed and run a business, etc
• Those who are engaged in agriculture and fisheries
• Those who are not enrolled in a company-based health insurance program
• Foreign nationals with visas with a validity of three months or longer who reside in Yao City

• お店などを経営している自営業の方
• 農業や漁業などを営んでいる方
• 会社の健康保険に加入していない方
• 3ヶ月以上在留資格がある本市在住の外国人の方

Please carry out the procedure within 14 days from the date of disqualification from the National Health Insurance System. 

(Documents necessary for enrolment) 

• One of any of the following
  
  • Certificate of Disqualification
  • A retirement certificate 
  • Proof of the final date of employment, such as an unemployment slip 
• Photo ID issued by public institutions (driver’s license, passport,residence card etc.)
• My number confirmation documents(House holder and all dependents)
• Certificate of designation, which is attached to your passport(if visa status is “Designated Activities”).

If a person from another household registers on your behalf, they must bring a power of attorney or equivalent declaration.

• Medical costs incurred up until the day benefit payments commence must be paid entirely by the individual.
• You will be required to pay insurance contributions retroactively up until the time you withdrew from your company’s Employee Health Insurance or other scheme (upper limit of two years)

• 下記のいずれか1点
  • 資格喪失証明書
  • 退職証明書
  • 離職票など退職した日がわかるもの
• 顔写真付きの公的機関発行の身分証（運転免許証・パスポート・在留カードなど）
• マイナンバー確認書類（世帯主と対象者全員分）
• パスポートに添付されている指定書（在留資格が　「特定活動」の場合）

※住民票上、別世帯の方が窓口にお越しいただく場合、委任状等も必要になります。

• 給付開始までの医療費は全額自費負担となります。
• 会社の健康保険等をやめた時（最長2年間）まで、保険料をさかのぼって納めていただくことになります。
